Supportive Housing Counselor - Dakota County

$38,110 base starting salary increased based on education/experience.
MHR is looking for candidates who are bilingual. Receive a $1000 salary bonus for passing the language proficiency exam!

If you have experience working in Adult Foster Care with adults recovering from mental illness, Adult Rehabilitative Mental Health Services (ARMHS), Independent Living Skills (ILS), residential mental health treatment (IRTS), MI/CD services, or other mental health services, you may be ready to take the next step in your career as a Supportive Housing Counselor!

This position provides housing transition and tenancy sustaining services to Dakota County residents to end or avoid episodes of homelessness and housing instability. Individuals served may have barriers to securing housing, including but not limited to, criminal history, poor rental history, and/or poor credit.

In this position you will:

  1. Coordinate integrated services and supports necessary to assist each person as they work toward their goal of recovery.
  2. To the extent requested by the individual, provide collaboration and education to significant others and family members who are interested in assisting the individual in their recovery process.
  3. Work in partnership with individuals to develop a plan of services and supports that will enable each person to live as independently in the community as possible.
  4. Ensure that services are implemented as developed by the individual and team and as they are documented in the treatment plan.
  5. Assist individuals with obtaining and maintaining benefits, finding employment or education, monitoring mental, chemical, physical, dental, and environmental health and making connections to appropriate providers, and assisting individuals in developing self-care and daily living skills.
  6. Complete all activities related to obtaining and maintaining housing such as educating individuals about tenant rights and responsibilities, advocating for rights, assisting in housing searches, coordinating with landlords, ensuring choice and access to appropriate housing options, and assisting with obtaining and maintaining housing subsidies.
  7. Ensure that all changes in client status or needs are communicated through documentation.

. Requirements:

Minimum Education and Experience

Bachelor's degree in social work/psychology or human services related field. Two years of experience in related positions or Associates degree with at least 6,000 hours of supervised experience in the delivery of mental health services; with at least one year experience in housing/issues of homelessness and community based mental health with adults recovering from mental illness or combination of equivalent experience and training.
